Release of Juvenile Dependency Division Information

SUPERIOR COURT OF CALIFORNIA, COUNTY OF SANTA CLARA
JUVENILE DELINQUENCY DIVISION

The purpose of this Order is to facilitate the provision of services to minors who are or have been wards of the court by authorizing the exchange of relevant information among members of the children's multidisciplinary services team which is engaged in the provision of integrated children's services as specified in Welfare and Institutions Code section 18986.40.

IT IS ORDERED as follows:
Information which is confidential by virtue of Welfare and Institutions Code section 827, may be exchanged regarding minors who are, or have been, wards of the court by members of the children's multidisciplinary team known as the Family/School Support Program under the following conditions:

(1) The information is relevant to the treatment and services available to the minor through the program and is limited to the information necessary to formulate a client's plan and to deliver services.

(2) The child's parent or guardian has signed the form, 'Consent for Participation and Limited Release of Confidential Information'.

(3) Every member of the multidisciplinary services teams who receives information on the children shall be under the same obligations and subject to the same confidentiality penalties as the person disclosing or providing that Creation. Every member shall sign an Oath of Confidentiality prohibiting disclosure of such information to unauthorized persons.
